In 2006, Birmingham Group Health Services, Inc. received its eighth consecutive

the highest level of accreditation that a rehabilitation service provider may be awarded. (CARF, The Rehabilitation Accreditation Commission, is an international accreditation body.)

Community Support Services

Birmingham Group Health Services, Inc.'s Community Support Services offer the highest quality, individualized care whenever, and wherever, it is needed.  Participants in Community Support Services will have access to services and supports that address functional impairments affecting abilities to live independently in their chosen community. The services focus on skill acquisition and mastery, environmental adaptations to support recovery and accessing resources.  Services are offered in locations that meet the needs of participants. Such locations may include individuals’ homes, hospitals, shelters, correctional facilities and other community locations.   In addition, specialized services are available to individuals affected with co-occurring disorders and individuals involved in the criminal justice system.

The Community Support Services program receives funding from the Department of Mental Health and Addiction Services and offers a wide range of services to those individuals affected by mental health and substance abuse issues, or both (co-occurring disorders).  These services include, but are not limited to, skill building, assistance with benefits, housing, advocacy and support, and linkage to other services providers that can appropriately address each individual's needs.  In addition, case managers work collaboratively with other Birmingham Group Health Services' departments such as Rehabilitation, Clinical Services, and The Umbrella domestic violence program, to improve each individual's quality of life and ensure access to a full range of services to aid in his/her recovery.


  HIV/AIDS Case Management

BGHS is the lead agency of a collaborative of Valley organizations providing services for individuals infected/affected by HIV/AIDS. Specially trained case managers meet with individuals infected/affected by HIV/AIDS to help address emergency and ongoing needs.  Special emphasis is placed on facilitating the development of trusting relationships between service providers and individuals that will provide the support and understanding needed to live independently with the HIV virus. 

 PREP Community Support Services

Birmingham Group Health Services (BGHS) receives funding from the Department of Corrections to facilitate the transition of individuals with mental health needs who are being released from correctional facilities into community living.  The Post Release Entry Program (PREP) assists these individuals with housing, benefits, and employment and provides links to appropriate mental health and substance abuse services.  With adequate supports, it is believed that individuals will be less likely to commit further crime.

Individuals participating in the PREP benefit from accessing the array of services offered by BGHS including the rehabilitation department.  The PREP Community Support Specialist collaborates with other service providers to assist in identifying and meeting the individual's needs.

Referrals to the Post Release Entry Program come from a variety of sources, including, but not limited to, state-wide correctional facilities, parole and probation officers, and transitional services via the Department of Corrections.

 Jail Diversion Program

Birmingham Group Health Services' Jail Diversion Program provides clinically appropriate evaluation and linkage with the Department of Mental Health and Addiction Services' (DMHAS) system and other community resources for court referred individuals.  Individuals referred to the program are pre-trial clients with symptoms of mental illness or current DMHAS clients for whom community care will provide an alternative to incarceration.

The Jail Diversion Program’s Forensic Team, consisting of a clinician and community support specialist, assess individuals at the Derby Superior Court.  The Team also works collaboratively with the Bail Commissioner to divert mentally ill offenders from incarceration to community care.

Participants in the Jail Diversion Program are provided with the following services: assessment, assistance in the development of conditional release plans, respite car and crisis management.
