Job Title: Community Outreach Worker
Department: Ryan White Program
Status: Part-time (15 hours)
Job Description:
Provide case management services to individuals infected, affected,
and at risk for
HIV+ and Persons with Aids. Provide direct service including:
intake, assessment of individual needs and coordination of services.
Maintain clinical records in accordance with grant requirements and
for the preparation of reports and statistical documentation for
program planning, funding, evaluation, and billing. Serve as a link
between consumer,
family, agency and community services referring consumer or family
members to
other community resources.
Requirements:
BA Degree in human services or related field; experience with
HIV/AIDS counseling a plus.

Job Description:
This is an exciting opportunity
to provide psychotherapeutic services to adults in an outpatient
clinic while receiving the support of our team of psychiatrists,
therapists, community support providers and rehab workers. Sixty
percent direct service; no after-hours or pagers. Excellent pay and
benefits.
Requirements:
LCSW or other licensure required. Bilingual
(English/Spanish) skills a plus.

Job
Description:
Young Adult Services Community Support Specialist- to
provide skills training and coordination of care with both onsite
and community based coverage: collaborate with a multi-disciplinary
team in treatment planning; assess individual situations and needs
regarding both mental illness and substance use; serve as link
between consumer, family, agency and community providing resources
and education; provide vocational support services as necessary and
maintain a flexible schedule to meet the needs of those in recovery.
Requirements:
Bachelor’s
degree in related field with minimum of three years related
experience.
